Position Summary
McLeod County currently has an opening for a Registered Nurse or Public Health Nurse. This rewarding career gives nurses the opportunity to work directly within our communities to educate people and give them the tools they need to improve health outcomes for themselves, for their family and their community. This position includes potential opportunities to coordinate and/or work in Family Health programs such as family home visiting, child and teen checkups outreach, disease prevention and control5/12/2, and child passenger safety. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned.
Supplemental:
- The 2025 salary range for a PHN is $28.22 to $42.33 and RN is $25.78 to $38.67 per hour depending on experience.

Essential Functions/Responsibilities
Position Duties
- Performs comprehensive individual and family assessment which includes health history, physical assessment, growth monitoring, developmental assessment, psycho-social assessment, assessment of family functioning, assessment for substance abuse or domestic violence issues, and assessment of basic needs including food, housing, income, resources and supports, and access to health care.
- Provides care coordination, case management, referral and follow-up to individuals and families who are member of a vulnerable population and/or high-risk groups.
- Develops relationships with parents and children to promote and support the growth of parent-child relationships, using an evidence-informed curriculum and infant mental health interventions.
- Documents patient assessment, intervention, care plan, and all formal and/or informal service data in medical records. Uses established medical record forms, databases and documentation practices.
- Collaborates in development and delivery of programs and activities for individuals, families and population groups that promote health and prevent disease.
- Participates in Department and community emergency response training and drills consistent with job classification in support of public health emergency and disaster preparedness.
- Performs other duties as assigned or apparent.
Knowledge, Skills, Abilities
- Ability to plan and carry out activities concerning public health nursing.
- Ability to apply nursing rules and regulations.
- Ability to perform comprehensive assessments of health status to determine the needs for care.
- Ability to identify possible problems and develop appropriate plans and follow up steps.
- Ability to deal effectively with clients and the public of various types of backgrounds.
- Ability to handle confidential information with tact and discretion.
- Knowledge of the principles and practices of general clinical and public health nursing.
- Knowledge of current trends in public health nursing.
- Knowledge of effective counseling and coaching techniques.
Minimum Qualifications
Education and Experience
- Current licensure as a registered nurse in the State of Minnesota, with associate degree and two years’ experience or equivalent.
- Valid Minnesota driver’s license
PREFERRED QUALIFICATIONS
- Experience in Public Health.
- Experience working within community setting.
- Bachelor’s degree from an accredited college with National League of Nursing-approved program in nursing.
- Public Health Nurse certificate.
Working Conditions
- Work is done in an office setting, community clinics, schools, client homes, and other community sites including, but not limited to, workplaces.
- Frequent travel may involve exposure to inclement weather conditions, unsanitary conditions in homes and unsafe environments.
Physical Requirements Of This Position
- Desk work involving use of a computer, telephone, and other devices.
- Communicate effectively with county staff, families needing protective services, county attorney, court personnel, and probation.
- Ability to read screens and documents.
- Apply to drive in various light and weather conditions.
- Ability to type and use a computer or mobile device.
- Office work may include reaching for documents, grasping documents, standing, walking and repetitive motions.
- Lift and carry files and material weighing up to 50 pounds.
