Seeking RN’s interested in a full time role at our Microsoft Living Well Health Center in Redmond.
The Crossover Health Nurse is an essential part of the collaborative patient care team that supports an exceptional patient experience. Our RNs are empowered to practice and contribute to the care experience. The RN actively manages and coordinates patient care by connecting patients with specialists and other services within our secondary network.
The RN always follows up with patients and guides them through the process and monitors them with kindness, caring, and efficiency. The environment is fast-paced and rapidly changing and the philosophy revolves around evidence based medicine guidelines.
- Provide Direct Patient Care and model key Crossover attributes of servant leadership, lead huddles, insure interdisciplinary care approach, following standards, demonstrated dyad and triad leadership model, and delivering great patient care outcomes.
- Encourage the care team is providing appropriate care in accordance with their duties and obligations.
- Practice evidence-based nursing care in collaboration with the patient-centered team to achieve optimum health outcomes while providing service excellence
- Coordinate and manage patient care with specialists and their facilities
- Management of population health tools with individual patient and group follow-ups
- Provide phlebotomy services
- Contribute to reporting requirements related to incident reporting and compliance
- Actively participate in the Practice quality improvement programs, as well as participate in multidisciplinary quality assurance programs that monitor the performance of the Practice according to written policies.
- Comply with all policies, procedures, and protocols of the Practice including assisting other care providers and extended care teams to comply and cooperate with clinical outcome reviews and measurements as may be established by the client.
Required Qualifications
Graduate from an accredited school of nursing with a minimum of 3 years of clinical , primary care experience. Outpatient care or urgent care experience desired
Valid Washington State Registered Nurse License
Current BLS ( Basic Life Support) certification.

Excellent follow-through with execution skills second to none
Proficient in using various computer applications including experience with EMR systems
Preferable experience with Apple and Google based products

Pay: $49.56 - $57.00 per hour
