Company Summary : Crossroads is an Arizona Department of Health Services licensed substance abuse treatment provider with proven expertise in serving men, women, and Veterans with Substance Use Disorder through the provision of high quality, successful, and affordable residential and outpatient recovery programs.
Due to federal contracts the following items are prohibited : Medical / Recreational marijuana, current parole or probation.
Job Summary : The Clinical Lead provides assistance to the Clinical Coordinator in maintaining organization of location specific clinical operations; aiding staff in completion of duties; assisting in all aspects of client services, including treatment planning, case load management, group / individual interventions, crisis management, coordination of care, and discharge planning.
Supervisory Responsibilities :
None
Essential Functions :
Maintains a client caseload, set by manager, completes all required documentation in a timely manner, including progress notes, evaluations, treatment plans, coordination of care, and discharge planning, all in accordance with regulatory requirements and Crossroads policies and procedures.
Assists Case Managers and Peer Mentors in completing their duties, handling specialized cases that require additional experience and resources, and taking lead on organizing coordination of care efforts.
Completes client intake and discharge processes including completion of comprehensive assessments and treatment plans.
Supports Clinical Coordinator in providing ongoing training to the clinical team when needed.
Provides individual and group intervention to include crisis management as required.
Facilitates coordination of care to include case review, weekly progress updates in treatment and coordination of aftercare services.
Participates in clinical staff meetings, trainings and other activities as required.
Discusses issues regarding program requirements and / or conflicts with clients.
Assist clients in completing applications for government assistance, probation / court obligations, and referrals to independent agencies.
Adhere to internal policies and procedures and behavioral health licensure, local, state, and federal rules and regulations.
Performs other duties, as required.
Qualifications :
Current CPR / First Aid / Narcan training.
Current and annual negative TB Test.
Current and valid Fingerprint Clearance Card.
Clean Motor Vehicle Record (MVR) check performed annually and upon hire, preferred.
Successfully pass a background check and an Arizona Department of Health Services Adult Protective Services check.
Must maintain high ethical standards and demonstrate sound judgment, problem solving skills, and strong organizational skills.
Knowledge, Skills, and Abilities :
A working knowledge of addiction, recovery, and twelve step programs.
Well-developed written and verbal communication skills and ability to interact effectively with internal and external audiences.
Must be proficient in Computer / Microsoft Office / EMR and typing.
Education & Experience :
High school diploma or GED required.
1-2 years related experience and / or training or equivalent combination of education and experience required.
Work Environment :
Fast paced behavioral health setting.
Primarily indoors, however, some outdoor work may be requested.
Physical Demands :
While performing job duties, the employee is regularly required to sit and speak or hear. The employee is occasionally required to stand, walk, use hands to handle or feel, reach hands and arms, stoop, kneel, crouch or crawl. The employee must occasionally lift and / or move up to 50 pounds and seldom lift and / or move >
100 pounds. Specific vision abilities required by this job include color vision, depth perception, and ability to adjust focus.
Shift Requirements :
Days, nights, weekends, and holidays.
Travel :
Valid AZ Drivers License.
May include travel between sites and other locations.
Local and statewide travel when requested or required.
