Job Description:
SUMMARY
Primarily responsible is to provide leadership and coordinate total nursing care for the patient in collaboration with the providers. Participates in patient and family teaching and provides leadership by working collaboratively with ancillary, nursing, and other patient care personnel. Maintains standards for professional nursing practice according to clinical standards, procedures and departmental objectives.
PRINCIPAL ACTIVITIES & RESPONSIBILITIES

Leadership responsibilities:
- You must be able to demonstrate and interact with Tribal members and the public in an empathic, courteous, professional, and efficient manner.
- Demonstrate excellent communication and empathic leadership
skills to provide ongoing mentoring and supervision of medical
assistants, phlebotomists, and team members.
- Develop and maintain clear roles and responsibilities with each team member.
- Participate in developing and leading rapid cycle
improvement projects to develop and standardize workflows.
- Develop, implement, and routinely monitor Standards of Practice (SOP)
- Conduct workflow and chart audits on a regular basis toward continuous improvement.
- Review and edit policy and procedures when needed to ensure compliance
- Independently problem solve and apply critical thinking skills to assist team members with developing and implementing workflows, processes, and supporting their technological needs.
- Effectively manage conflict to build a trust-based team.
- Create an effective trust-based team that works towards metric driven outcomes.
- Onboard and assess/train skills for Medical Assistants
- Actively lead meetings (e.g. daily huddles, weekly meetings with MA/ancillary staff) with the clinical teams.
- Participate in committees and weekly meetings at various levels.
- Provide excellent customer service to clients and vendors of Three Rivers Health Center.
- Maintain a good attendance record, vacation requests, timecards.
- Develop and maintain Employee Maintenance Tracking sheets (e.g. learning management, CPR recertification dates etc.)
- Participate in assisting the development of and maintenance of a robust Quality program/Reporting
- Participate in the development of and support a robust Wound Care Clinic
- Maintain appearance standards as outlined in CTCLUSI policies.
- Perform Medical assistant and phlebotomy reviews.
- Possess knowledge of common safety hazards and precautions to establish/maintain a safe work environment.
- Possess knowledge of CPT coding to assist the team as needed under the directions of Revenue Cycle team

Clinical responsibilities:
- Work closely with the Clinic Coordinator to maintain clinic flow (e.g. staffing, workflows)
- Ordering and maintaining vaccines, medication, and supplies using TRHC resource management systems.
- Be proficient in electronic medical record to help team with standardization of workflows, adhere to quality workflows and in general support EMR tickets and other needs as they arise.
- Support professional nursing practice across practice settings
and across the continuum of care to meet the needs of the
patient and family.
- Possess knowledge of professional nursing theory, regulations and practices to give and evaluate patient care.
- Possess skill in patient triage in person and on the phone.
- Provide all patient care in a manner that reflects respect for patient rights, dignity, values, culture preferences, and expressed needs
- Delegate patient care according to skill level, experience, patient acuity, fiscal accountability, and adequacy of resources.
- Possess skill in initiating appropriate crisis interventions and emergency response.
- Possess skill in preparing/maintaining records, writing reports and responding to correspondence.
- Possess the ability to calculate and administer drug dosages and injections and measure results.
- Possess the ability to maintain quality control standards.
- Possess the ability to react calmly and effectively in emergency situations.
- Assess patient's physical, psychological, and social needs
- Assess pain and implement appropriate pain management techniques
- Develop, implement, and revise care plans that reflect patient's diagnosis and standards of care
- Involve patients and/or significant others in decisions regarding care
- Document patient history, symptoms, assessments, tests, and care according to guidelines
- Review patient history, diagnostics, and lab data, and report any abnormalities
- Educate patients and families on managing injuries and illnesses to ensure a safe and successful discharge
- Accountable for the coordination of nursing care, including direct patient care, patient/family education and transitions of care.
- Skill in applying and modifying the principles, methods and techniques of professional nursing to provide ongoing patient care.
- Ability to analyze options and then counsel patients and families about choices and make referrals to other providers and resources.
- Ability to engage clients and families in the development and implementation of a care plan.
- Ability to read and interpret physicians’ orders and notes from other providers.
- Other duties as directed by management.
PHYSICAL & MENTAL DEMANDS
- Must be able to handle and work with standard nursing equipment and supplies including thermometer, blood pressure monitors, scales, and syringes. May involve use of assistive devices such as walkers, wheelchairs, ventilators, pulse oximetry units, and oxygen tanks.
- Must be able to walk, talk, hear, use hands to handle, feel or operate objects, tools, or controls, and reach with hands and arms.
- Combination of office and exam settings. Frequent exposure to medicinal preparations and other conditions common to a clinic environment.
- Vision abilities required by this job include close vision and the ability to adjust focus.
- May be required to push, pull, lift, and/or carry up to 40 pounds.
- Must be willing and able to work long hours, which may include evenings and weekends.
- Must be willing and able to travel both locally and within the CTCLUSI service delivery area.
WORKING CONDITIONS & ENVIRONMENT
- Varied activities including standing, walking, reaching, bending, lifting. Requires full range of body motion including handling and lifting patients manually.
- The job requires standing and walking for extensive periods of time.
- The job occasionally requires lifting and carrying items weighing up to 100 pounds.
- The job requires good finger dexterity and good hand-eye coordination.
- The job requires corrected vision and hearing to normal range.
- May be required to work in locations other than Three Rivers Health Center, such as schools or at an outreach work location.
- May be required to attend meetings, conferences and Tribal events.
