- Certifications:** BLS, Non-Violent Crisis Intervention Course (8 hours), CPI or PMCS Certification
- Must-Have:** EPIC experience highly preferred
- Deliver patient care through the professional practice model of Relationship Centered Care, applying nursing process based on theory, research, evidence-based practice, and approved organizational nursing standards.
- Assess and evaluate patient care needs, applying critical thinking skills in patient care management.
- Integrate relevant assessment and intervention skills in nursing care delivery.
- Collaborate with interdisciplinary healthcare teams in the development, implementation, and evaluation of individualized care plans.
- Prioritize patient care, including teaching, rounding, coaching, planning post-hospital care, and appropriate delegation.
- Educate patients and caregivers regarding the plan of care, health promotion, and disease prevention.
- Communicate effectively and professionally with patients, family members, and the Interdisciplinary Patient Care Team.
- Manage patient care assignments using the Care Delivery Model of Modified Primary Nursing.
- Role model Relationship Centered Care actions, including Admission Welcome, Bedside Shift Report, Purposeful Hourly Rounding, and Discharge Wrap Up.
- Ensure safe and efficient patient-centered care by identifying and addressing barriers and opportunities for improvement.
- Implement process improvement strategies, engaging in peer and institutional review processes.
- Collaborate with teams to implement quality improvement projects and nursing research studies.
- Demonstrate patient-focused, cost-effective approaches to care regarding equipment, staff, supplies, and other resources.
- Accurately document nursing practice in compliance with regulatory and legal requirements.
- Support a care environment conducive to clinical colleagues, patients, families, and healthcare team members.
- Encourage and engage in scholarship, inquiry, learning, and innovation for professional development.
- Participate in shared decision-making through practice councils, nursing committees, and unit initiatives.
- Education:** Bachelor of Science in Nursing preferred, an associate degree considered with relevant experience and/or active enrollment in a BSN Program. English proficiency required.
