What are the responsibilities and job description for the Part Time Licensed Practical Nurse (LPN) position at DDMS LLC?
Fern Park is looking for a few part-time, caring and compassionate LPN's to assist our residents with intellectual disabilities in our homes. You will provide nursing health care services to individuals according to policies and procedures and within the standards of the individual states Nurse Practice Act, ICF/DD and individual State Standards.
RESPONSIBILITIES:
1. Assists in the implementation and continuing evaluation of each individual’s Active Treatment Plan.
2. Participates in the ongoing communication process, verbal and written of individual health and health care intervention.
3. Maintains and assumes accountability for all scheduled and controlled substances as defined by facility policy and pharmacy regulations.
4. Maintains security and cleanliness of each medication room.
5. Assists in maintaining adequate drug and pharmaceutical stocks to provide continuity of medicine and Enteral feeding administration.
6. Transcribes physician’s orders.
7. Assist in implementing training for personal hygiene, skin care, and self-medication programs.
8. Actively contributes as needed to employee in-services, training and education.
9. Attends scheduled training and in-services demonstrating learned competencies.
10. Completes internal monitoring tools as assigned.
11. Completes internal investigation of unusual incidents as assigned.

This position has access to protected health information (PHI) for the purpose of providing service and supports to individuals in the facility and has authorization to disclose PHI as outlined in company policy and procedure. Any unauthorized use or disclosure of PHI will result in disciplinary action, up to, and including termination of employment.
