Wound Care Nurse – Venza Care at Decatur Rehabilitation and Healthcare Center!

Shifts :

  • Day Shift (Monday – Friday)

    • Key Responsibilities :

  • Assess, develop, and implement individualized wound care treatment plans for residents.
  • Monitor and document progress of wounds , adjusting care plans as necessary.
  • Perform wound debridement, dressing changes, and infection control measures .
  • Collaborate with nursing staff, physicians, and interdisciplinary teams to ensure comprehensive patient care.
  • Educate staff, residents, and families on proper wound care, prevention techniques, and treatment procedures .
  • Ensure compliance with state, federal, and facility regulations regarding wound care .
  • Maintain accurate records and reports, documenting wound assessments and treatment outcomes .
  • Assist with pressure ulcer prevention programs and skin integrity assessments.
  • Participate in quality assurance initiatives to improve wound care outcomes.
  • Serve as a resource and mentor for nursing staff regarding best practices in wound management.

    • Qualifications :

  • Valid LPN or RN license in the state of employment.
  • Wound Care Certification (WCC, WOCN, or similar) preferred.
  • Prior experience in wound care, skin integrity, or related nursing field preferred.
  • Strong clinical assessment and critical-thinking skills .
  • Ability to work independently and collaborate with interdisciplinary teams .
  • Excellent documentation and communication skills .
  • Commitment to compassionate, patient-centered care .
