What are the responsibilities and job description for the Prep and Profile Specialist II position at Diakon?
- Responsible for serving as the child preparation and profile specialist for the Permanency Services program.
- Conducts home and/or office visits with families, children or other professionals.
- Meets with and gathers relevant information from birth families, resource families, other professionals and the children to assist in the development and completion of the child profile.
- Provides post adoption services as needed.
