What are the responsibilities and job description for the ACT Services Lead RN position at District 19 Community Services Board?
District 19 CSB is seeking a qualified professional to work as an ACT Lead Nurse for the interdisciplinary ACT (Assertive Community Treatment) team that serves adults with severe and persistent mental illness in the Petersburg/Tri Cities area.
This member of the multidisciplinary team is responsible for: conducting psychiatric assessments, assessing physical health needs, providing administration of intramuscular medication, obtaining lab work and following-up with results, providing care coordination between various service providers in the community, maintaining accurate documentation (including audits of staff) and accuracy of ACT MDR, coordinating medication deliveries and communicating with local pharmacies to ensure accuracy of medication deliveries, writing appropriate referrals to community physicians, and providing a range of treatment rehabilitation and support services.
This position would serve as the lead nurse, providing oversight of the team’s medical services, under the direction of the psychiatrist, supervising psychiatric nurses, and sharing shift management responsibilities with other staff.
Position provides community-based psychiatric treatment and will require entering homes and/or community centers, medication delivery, medication education and monitoring activities, daily living assistance, injections, and family support and case management.
Successful applicant must be licensed as a registered nurse in the state of Virginia. Must have a minimum of an Associate degree in nursing. Applicant must also have at least one year of work experience in an inpatient or outpatient psychiatric setting.
Must be able to provide phlebotomy services, administration of intramuscular medication, supportive counseling, have knowledge of psychotropic medication uses, side effects, and potential interactions. Knowledge of community-based services required as well as some out of office work.
Travel to consumer’s residences required, with agency vehicle provided or mileage reimbursement for personal vehicle.
Must have a current State of Virginia license to practice as a Registered Nurse. Required to obtain and maintain CPR certification.
Must have the ability to tolerate light lifting, bending, stooping, reaching, and climbing stairs.
Salary:
Position is a Grade S-C11. The starting salary is $66,048.00. Salary is negotiable.
Hours:
ACT is an essential program, operating 365 days of the year. Applicant must be able to work some evenings (until 6:00pm), weekends, and holidays, based on program needs.
