What are the responsibilities and job description for the LPN position at Easterseals Arc of Northeast Indiana?
Summary: LPN performs a wide variety tasks which are broad in scope, and involve many complex and significant variables. The LPN functions in a task-oriented role under an RN’s direction. LPN works according to standard agency policies and procedures and standard practices of the profession, exercising independent judgment to implement risk plans and solve problems. LPN works with moderate supervision, referring to supervisor unusual or unprecedented situations. LPN maintains frequent contact with participants, agency staff, other service providers and the public for the purpose of exchanging information, medical coordination and solving problems.
Duties and Responsibilities:
- Maintain compliance with all regulatory bodies
- Performs nursing duties and provides client nursing care in accordance with The Indiana Nurse Practice Act, MedicaidWaiver standards, ISO standards, Indiana State Department of Health standards, and Easterseals Arc of NE Indiana policies and procedures.
- Completes required data reviews and updates nursing leadership with any concerns
- Meets Balanced Scorecard Goals including billable time
- Ensure services provided are individualized
- LPN will have client caseloads consisting of clients on Medicaid Waiver and in Group Homes.
- Completes routine monitoring, consults and face to face visits for assigned individuals with wellness Tiers 1,2 and 3
- Provides training to staff on medical procedures and risk plans.
- Maintains client medical/nursing charts. Documents medical/nursing interventions and contacts.
- Completes assigned Quarterly assessments for Wellness and Group Home Individuals and required paperwork by deadlines
- Performs emergency care to clients and staff as needed until emergency responders arrive
- Support people to be safe and have optimal health and well being
- The LPN provides direct care; functions as a member of the healthcare team; provides input into the assessment, planning, and evaluation of nursing care; and implements the plan of care under the RN’s direction.
- Develops and adjusts risk plans for assigned caseloads and provides initial training over risk plans.
- Provides in-service training to Easter Seals staff, clients and family members including orientation training
- Assists with medical appointments as needed such as gathering paperwork, obtaining referrals, etc.
- Is part of on call rotation. Follows department on call policies.
- Positively represent Easterseals Arc.
- Communicates with individual’s care team regarding medications, obtaining prescriptions and updates EMAR with necessary changes participates in cycle fill
- Manages and coordinates client health care needs by collaborating and communicating with other health care professionals (physicians, dentists, psychiatrists, medical specialist, dietitian, pharmacy, laboratory, etc.) and with the interdisciplinary team and staff involved with the client’s health care.
- Assists with transitions and discharge of individuals
- Adheres to Easter Seals Arc Human Resources and EEO/AA policies
- Performs other duties as assigned.
Qualifications:
- At least 18 years of age
- Current Indiana licensure as an LPN required.
- 1 year of experience in ID/DD or long term care nursing preferred
- Ability to communicate effectively in order to: Complete required forms and reports of services provided, Follow oral and written instructions, and Communicate with an individual in the individual’s mode of communication
- Ability to pass criminal/registry background checks, A valid driver’s license & insured vehicle
- Ability and willingness to support individuals with challenging behavior and/or medical concerns
- Ability to pass and maintain required trainings such as CPR/First Aid, Medication Administration and Positive Behavioral Support Training and other trainings as deemed necessary by the agency
- Ability to perform moderate to heavy lifting when needed
- Ability to train staff, clients and family members on health related topics
- Willing to travel primarily within Allen County and occasionally the surrounding counties Easterseals/Arc serves with reimbursement for mileage
