Responsibilities of a Social Worker:
- Conduct assessments to identify the needs of residents and develop individualized care plans.
- Provide counseling and support to residents and their families.
- Advocate for residents’ rights and ensure their voices are heard.
- Coordinate with healthcare providers, community resources, and family members.
- Facilitate support groups and educational sessions for residents and families.
- Assist with discharge planning and follow-up services.
Qualifications of a Social Worker:
- Must be a licensed Social Worker in PA
- Responsible, ready, and eager to work
