About the Program:
El Dorado Center (EDC) is a 16-bed co-ed community-based, short-term crisis mental health residential treatment program for individuals who may be stepping down from locked psychiatric care. An intensive, structured residential program, EDC is an unlocked, home-like environment facilitating the healing process in preparation for transitioning back to community living. Our staff provide individual and group counseling, crisis intervention, structured activities, community outings, and assistance with independent living skills and connecting to the community. A nurse is on staff and a county psychiatrist is available for medication monitoring and medical care coordination.
About the Position:
We currently have a full-time (up to 40 hours/week) opening for a Nurse (LVN or RN) with our EDC program. Pay is dependent on experience and licensure ranging from $30-40/hour for an LVN and $40-$50/hour for an RN. Hours are Monday through Friday between business hours. There is an option for four 10 hours shifts, if preferred. This position does not require being on-call after-hours, weekends or holidays.
Under supervision of the Director of Nursing and Health Services, the EDC nurse provides direct skilled nursing services within scope of practice to residents seeking behavioral health services. The nurse in this position performs ongoing assessments, evaluations and triage, supports with intakes and discharges, and provides health related education to residents and staff. The nurse collaborates with multi-disciplinary team members within the organization and with multiple healthcare entities throughout the community.
Primary Responsibilities:
1. Performs basic nursing services within scope of practice, such as: assessments, support in primary care provider (PCP)/dental care establishment, and follow-up. Supports with referral coordination.
2. Assists clients in improving and maintaining stability by providing medication support, education and monitoring.
3. Ensures accurate implementation of prescribing provider orders, as well as safe adherence to medication protocols.
4. Works closely with Medication Specialist and other identified trainers in supporting staff with medication procedure training.
5. Participates in monthly medical team meetings and supervision.
6. Adheres to required infection control and aseptic techniques in the delivery of resident care.
7. Communicates closely with onsite/remote county psychiatric providers and community healthcare providers to deliver integrated care.
8. Observes, records and reports signs, symptoms, and behaviors including the physiological and psychological status of residents.
9. Notifies appropriate supervisory staff of changes in residents’ status; recognizes care priorities and potential crises.
10. Facilitates weekly client health groups.
11. Provides assessment, education and linkage to residents looking to access MAT treatment programs.
12. Exhibits non-judgmental, empathetic and supportive approach when communicating.
13. Consistently uses clear, concise, and effective written and oral communication; complies with departmental policies when sharing/documenting relevant resident care data; promotes strict resident confidentiality and right to privacy.
14. Works with the pharmacies and providers in obtaining medication orders, prescriptions, refills, and prior authorizations.
15. Consults with provider whenever necessary in order to advise resident of most appropriate action; reinforces provider’s plan of care.
16. In case of referral to the Emergency Department or another facility, coordinates care with appropriate staff at the other facility, including nurse-to-nurse reports.
17. Provides POC testing for COVID, HCV, HIV, and syphilis
18. Other responsibilities as assigned by supervisor.
Qualifications:
Required:
• Successful completion of a nursing program.
• Unencumbered and unrestricted license to practice as an RN or LVN within the state of California.
• Current BLS for healthcare provider’s certification.
• At least 1-year nursing experience.
• Must have a health examination including a screening for tuberculosis within 90 days prior to employment or within 7 days after employment and at least annually thereafter.
• Competent in basic computer skills.
• Demonstrated skills in effective written and verbal communication.
• Ability to recognize the needs and concerns of diverse groups of people.
• Must be able to pass a criminal record clearance.
