What You’ll Do:
- Support consumers in achieving personal care goals and addressing ongoing challenges
- Ensure adherence to Adult Mental Health Rehabilitation Services (AMHRS) standards
- Respond promptly to consumer concerns, ensuring high satisfaction
- Educate consumers on medications, track logs, and conduct monthly audits
- Coordinate services based on individual needs
- Deliver high-quality care in line with Individual Rehabilitation Plans (IRP)
- Accurately document services within required timeframes using electronic health records
Job Requirements:
- Bachelor’s Degree in Mental Health or Human Services, OR
- A combination of 1 years of college and related life or work experience totaling 4 years, OR
- High School/GED plus 4 years of related work or life experience
- Intermediate computer skills with the ability to learn new programs
- Valid Driver’s License and an acceptable driving record
- Strong verbal and written communication skills
- Compassionate, flexible, and client-focused approach
