Summary: 

Incumbent in this position will provide transportation and support to and from medical appointments for members in Community Living Services programs. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. 

Essential Responsibilities, include but are not limited to:

· Transport and accompany members to appointments and other services, including work centers as needed.

· Assist in preparing information related to upcoming appointments.

· Report back to nursing staff following any appointments, including completion of any paperwork pertaining to the appointment.

· Assist in ensuring nursing CBTA and member clinical paperwork is completed and filed as needed.

· Supervise and assist members in taking their medication(s) in accordance with program guidelines as needed before and after appointments.

· Respond to medical and behavioral emergencies following appropriate Health and Safety, First Aid/CPR and other procedures as necessary.

· Monitor and assist members with personal grooming and appearance as necessary.

· Immediately report any medical/behavioral issues or emergencies or other unusual incidents to appropriate nursing and program staff.

· Position may be required to work nights and weekends.

· Will be required to perform other duties as needed.

QUALIFICATIONS 

To perform this job successfully an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required.

EDUCATION, SKILLS, and/or EXPERIENCE

o High school diploma or equivalent (must present physical copy upon hire date)

o Knowledge of medical terminology

o DSP certification preferred but not required

o Must have a valid drivers license
