Clinical Admissions Liaison (COTA/PTA) Summary
- FIELD BASED POSITION PRIMARILY WORKING WITH REFERRAL SOURCES BASED IN DUTCHESS, PUTNAM, & NORTHERN WESTCHESTER.

The Clinical Admissions Liaison is a field-based position that acts as a liaison between prospective residents/patients, their families, various referral sources (hospitals, physicians, management care organizations, physician groups and other related agencies), and the facilities they represent.
- Clinical Admissions Liaison (COTA/PTA), Certified Occupational Therapy Assistant/Physical Therapy Assistant, acts as the facility liaison with all referral sources such as hospital discharge planners and related rehabilitation unit(s), physicians, health care agencies, health maintenance organizations, insurance companies, and any other source of referral.
- Ability to establish and maintain strong working relationships with hospital discharge planners, case managers, social workers, nurses, physicians, and other members of the healthcare professional community.
- Clinical Admissions Liaison (COTA/PTA), Certified Occupational Therapy Assistant/Physical Therapy Assistant must maintain a positive rapport with families and referral sources to foster trust and ongoing referrals.
- Responsible for the review, selection, assessment, and coordination of all Residents/Patients’ medical and related information necessary in determining appropriateness of placement/discharge planning, help meet census goals, and obtain the necessary and required authorizations/approvals for placement.
- Clinical Admissions Liaison (COTA/PTA), Certified Occupational Therapy Assistant/Physical Therapy Assistant must provide prospective residents/patients and their families clear communication about services, care plans, and facility amenities.
- This requires travel to various referral sources as needed or directed.
- Responsible to strengthen current and build new ties to the community, enhance relationships with hospitals, senior living facilities, local senior centers, home care agencies, and physician’s offices.
- Clinical Admissions Liaison (COTA/PTA), Certified Occupational Therapy Assistant/Physical Therapy Assistant works in conjunction with the facility’s Admissions team, Administrator, Director of Nursing, Director of Therapy/Rehabilitation, the Medical Director, Attending Physicians, and other department heads involved in admission, care, and discharge of resident.
- Conduct on-site evaluations (e.g., in hospitals or patient homes) to assess suitability for nursing home placement.
- Facilitates admission decisions and ensures a positive experience for each resident and their family. This may include executing admission agreements and determining whether admission is short or long-term and establishes projected length of stay.
- Conducts the necessary reviews of any and all referrals, PRI’s and/or medical records for all aspects of medical care and rehabilitation services.
Requirements
- Associate’s Degree/Two-Year College Program for Physical Therapy Assistants or Occupational Therapy Assistants.
- Currently licensed and in good standing in NYS as a Physical Therapy Assistant (PTA) or Certified Occupational Therapy Assistant (OTA/COTA).
- At least (3) years experience in areas such as rehabilitation, sub-acute units(s) or an acute care hospital.
- Experience in marketing and/or public relations, preferred or sales/marketing background in a healthcare capacity, a plus
- Excellent clinical assessment skills with a proven ability to relate to all levels of professionals, along with the ability to manage a fast-paced often intense daily routine with multiple priorities and time frames.
- Self-motivated and driven can-do attitude.
- Upbeat and outgoing personality, with approachable nature.
- Valid Driver’s License and reliable transportation
- Familiarity with the admissions process at a sub-acute rehabilitation facility.
- Excellent organization skills, time management skills, and attention to detail.
- Ability to work well under pressure and be flexible.
- Exemplary customer service and communication skills.
- Willingness to travel to various referral sources, within the Middle and Lower Hudson Valley Region. Predominantly within Dutchess, Putnam, and Westchester Counties up to 100% of the time.
