Mental Health Division
Administrative Case Manager - Geriatric division position available for Mastered Level (Social Work preferred) Mental Health Professional. This position will provide case management services to geriatric persons with serious and persistent mental illness, as well as provide consultation and educational support to nursing care facility staff.
Specific Duties Include
1) Complete assessments of geriatric consumers on local inpatient units, in their own homes, or at other community sites. Provide access and support to persons with serious mental illness at or in need of nursing home or personal care home placement.
2) Develop goal plans which are complementary to the clients overall care plan at the nursing care facility or personal care home placement. This will be accomplished through meetings with the clients, as well as regular attendance at care plan meetings at the facilities. Goal plans should maximize self direction, as fully as possible, based on the individual person’s unique needs, preferences and experiences.
3) Be knowledgeable of the regulations which govern nursing and personal care facilities and reflect this knowledge by performing case management, consultation, and education supportive services which are consistent with those regulations.
4) Assist facility staff in the coordination of services with consumers, providers, doctors, family members, and others, as appropriate.
5) Assist and support the Warren State Hospital/Geriatric Specialist, as needed, providing case management and assessments to specialized populations.
6) Serve as outreach, education and resource person to professionals in the Erie community on issues related to mental health and aging.
7) Conduct intake duties in all areas of Administrative Case Management, as needed.
8) Other duties as assigned.
QUALIFICATIONS
Master’s Degree in Social Work, Psychology, or related field, and at least one year of mental health experience. Professional licensure preferred.
