General Description:

Responsible for performing all duties/strategies related to ETCADA Transition program as a Case Manager. Duties primarily consists of assisting at risk youth/young adults ages 15 1/2 to 25 with barriers impacting their ability to live stable, successful lives. Barriers include but are not limited to victims of crime resulting in trauma and/or current/former foster care populations. The Case Manager will assess participant’s needs, support and facilitate individual transition plans to meet those needs; including goal setting and providing tools needed to achieve successful outcomes. Plans can include individual and group counseling, facilitation of peer support groups, referrals to community resources, employment services, job ready training, career development, advocacy, assistance with transportation, housing assistance and financial literacy.

Job Duties and Responsibilities:

  • Develop on-going collaborative relationships with youth oriented and other supportive service organizations where referrals for services may originate or be referred to Such as DFPS, probation, parole, high schools, judges, counselors, Work Force Solutions, colleges and universities, legal aid, LPC’s, FQHC’s, Literacy Council, LCDC’s, mental health professionals, substance abuse professionals etc.
  • Execute in a timely manner all appropriate paperwork when determining participant’s eligibility.
  • Maintain the highest level of confidentiality ensuring all consents are accurate and fully adhered to.
  • Develop healthy boundaries and relationships in order to provide the best assistance possible.
  • Implement the principles of trauma informed care consistently. Provide for the safety and security for all participants.
  • Implement best practices with; case management, Court Advocacy, Job readiness, mentoring and community advocacy.
  • Document completely, timely and accurately in each participant file according to regulations and best practices ensure reporting to funders meets highest standards.
  • Follow all policies, procedures, laws, and professional guidelines and recognize where it is appropriate to contact administration for direction prior to action.
  • Maintain all professional licenses and continuing education hours to maintain a level of expertise that ensures the best care for the participants.
  • Conduct or participate in case management review meetings to ensure strategic alignment with all cases being on target for success.
  • Assist foster youth when referred for case management: currently in foster care, aging out of foster care, formerly in foster care and in need of assistance.

· Assist youth when referred for case management: on probation, parole, youth released from juvenile correctional facilities.

· Assist youth in contacting employers for employment opportunities and guide them to maintain employment.

  • Assist youth with creation of resumes, job search, job application, career assessment and perhaps job training.
  • Assist with operations and maintain a pleasant, cohesive work environment that is welcoming to all participants and visitors.
  • Ensures Participants are actively engaged in accomplishing Transition Plan goals and updates Transition Plans as needed.
  • Participate in community positive activities.
  • Travel in Council service area and work a flexible schedule as required for fulfilling contract requirements.
  • Ensure that all grant activities/performance measures are carried out as stated in the approved contract.
  • Ensure that documentation necessary for expenditure of contract funds is accurate, complete, and submitted on time. (Time sheets, travel report, requisitions, etc.)

· Must keep work area, storage of work supplies in a neat and organized manner without clutter or excess items in and around the desk area.

· Liaison between the administration, community partners, participants, and special identified needs that need to be met.

· Facilitate Peer support groups on various topics as needed.

Education and Training Requirements:

  • Bachelor’s degree in a behavioral science. Commensurate experience may be considered.
  • Experience in working with CPS, Foster Care and/or Texas Workforce preferred, case management, youth services.
  • CPR/First Aid (within 60 days of hire or contract start date)
  • Suicide Prevention Training (one each year)
  • Complete Mental Health First Aid and trauma Informed care each year
  • Complete grant specific trainings according to contract and all ETCADA trainings.

Characteristics/Experience Preferred:

  • Experience in youth advocacy for at-risk youth helpful; serving with compassion and a non-judgmental approach a must.
  • Must be able to develop and maintain rapport with young people.
  • Experience in progressively responsible positions in comparable environment or in youth related field work preferred.
  • Computer proficiency required.
  • Ability to work under minimal supervision, self-driven, good judgement, strategic.
  • Experience in grant program activities and reporting preferred.
  • Excellent oral and written communication skills.
  • Strong interpersonal skills, able to work as a team member and with other community professionals.
  • Knowledge of trauma informed care and strategies.
  • Ability to plan, organize, prioritize and multi-task a must.
  • Working knowledge of community resources and contacts.
  • Reliable transportation and good driving record
  • Experience in facilitating peer support groups on various topics preferred.

WORKING CONDITIONS:

· Work may be performed in an office environment, extensive field work meeting with participants where they live and work. Assisting with transportation, transport clients when necessary to appointments. Travel in and out of service area (which may require out of town/state overnight or more stays) and work a flexible schedule as required for training requirements.

  • Maintain, clean driving record, criminal record, unrestricted driver’s license, auto insurance at all times. Immediately reports to supervisor in writing any deviations from the above.
  • Abides by and is knowledgeable in all ETCADA P&P, Grantor/ State/Federal regulations.
  • Must be able to lift a minimum of 25 pounds, bend, stoop, walk, drive and sit for long periods.
  • Exposed to weather elements.
