Evergreen Life Services is Hiring a Nurse Practitioner Near Antioch, TN
- Order, interpret, and evaluate diagnostic tests to identify and assess patient's clinical problems and health care needs.
- Record physical findings, and formulates plan and prognosis, based on patient's condition.
- Discuss cases with physician and other health professionals to prepare comprehensive patient care plan.
- Submit health care plan and goals of individual individual(s) served for periodic review and evaluation by physician.
- Prescribe or recommend drugs or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures.
- May refer individual(s) served to physician for consultation or to specialized health resources for treatment.
- Maintain current knowledge of each individual’s medical condition.
- Participate in Interdisciplinary Team meetings as required.
- Serve on committees as required
- Regular and punctual attendance
- Keep staff well informed regarding health-related problems of the persons served; discussing reasons for treatments, medications, etc. with the appropriate parties
- Provide in-service training to staff on medically-related issues
- Ensure that all policies, procedures, regulations, and guidelines are followed per state and agency requirements
- On-call availability for medical concerns
- Graduate from an accredited school of Nursing. Must possess and maintain current registration.
- Master’s degree from an accredited advanced Nurse Practitioner program.
- Successfully pass the Certification Exam Experience
- Minimum of one (1) year clinically relevant experience is required. Experience working with elderly, or intellectually disabled preferred.
- Must have knowledge of and technical skills of medical nursing procedures, first aid, and basic principles of Nursing. Must know or learn about individuals with intellectual and developmental disabilities (IDD), seizure disorders, Autism, and other associated diagnoses pertaining to the field of intellectual disabilities and geriatrics.
- Working knowledge of Windows 7, Office 2010 applications. Aptitude to learn other software programs as required for this position.
- Occasional travel

- Regularly works in fast pace environment with multiple task deadlines
- Regularly moves and positions objects weighing up to 50 pounds
- Occasionally exposed to viruses and infectious conditions
- Ability to meet the public in a manner that reflects creditably upon the organization and must have the ability to deal with and staff in a manner that is conducive to harmony and their best efforts
- Must be adaptable and have the ability to make decisions
- May be required to attend seminar or job-related training courses
- Must have understanding, patience and tact in dealing with individual(s) served, their families or advocates and other agencies involved in providing supports for individual(s) served
- Must have the ability to maintain good working relationships with Evergreen staff and with contacts from other agencies or entities
- Must be able to prioritize work tasks
- Must be able to work without close personal supervision
- Will not supervise.
