Description
Supervisor: Clinical Manager
Job Purpose:
The Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN) is responsible for providing
care/services to assigned patients under the supervision of a Registered Nurse and in accordance with
the patient’s plan of care.
Responsibilities:
1. Provides care/services to patients including the administration of treatments and medications
according to the patient’s plan of care.
2. Prepares clinical and progress notes.
3. Maintains accurate and timely documentation of care/services provided.
4. Reports any changes in the patient’s condition or plan of care to the Registered Nurse.
5. Participates in the nursing process by evaluating and making recommendations for revisions to
the plan of care, as appropriate.
6. Only performs treatments or special procedures as authorized by the Home Health Agency and
deemed competent to perform by the Clinical Manager.
7. Prepares equipment and materials for treatments observing aseptic technique, as required.
8. Assists the patient in learning appropriate self-care techniques.
9. Demonstrates competency in providing patient care/services and provides care/services in
accordance with professional standards and the state’s nurse practice act.
10. Provides appropriate patient education and teaching materials.
11. Reviews the patient’s medications; updates the patient’s medication profile when there are
new, changed, or discontinued medications; and notifies the Registered Nurse of medication
changes.
12. Communicates with other team members, as appropriate, regarding the patient’s status.
13. Attends and participates in staff meetings, educational in-services, and committee meetings, as
appropriate.
14. Participates in the on-call rotation as requested.
15. Reports adverse events, incidents, and grievances/complaints (patients and/or personnel) to
the Clinical Manager.
16. Documents the patient’s progress and effectiveness in meeting goals and outcomes.
17. Participates in the patient’s discharge planning.
18. Participates in the agency’s Quality Assessment and Performance Improvement (QAPI)
program, as appropriate.
19. Adheres to the agency’s policies and procedures.
20. Maintains confidentiality of all aspects of job performance.
21. Practices proper infection control and utilizes PPE as indicated.
22. Complies with all applicable federal, state, and local laws and regulations in performance of job
functions.
23. Performs other related functions as directed.
Physical Demands:

1. The employee is regularly required to stand, walk, and sit, as well as talk and hear.
2. The employee is required to use hands to operate vehicles and office equipment.
3. The employee must occasionally lift and/or move up to 30 pounds.
4. Specific vision abilities required by this job include close vision, distance vision, peripheral
vision, and the ability to adjust focus.
Qualifications
1. A graduate of an approved school of nursing who is licensed as an LPN/LVN by the state of Indiana.
2. At least one year of recent nursing experience preferred.
Training:
Home health care experience preferred.
