Key Responsibilities:

• Perform in-home patient assessments and develop personalized care plans in coordination with the home health agency team.
• Administer medications, IVs, and other prescribed treatments.
• Monitor patient progress, provide updates to the supervising physician, and adjust care plans as necessary.
• Educate patients and their families on managing health conditions, medications, and care techniques at home.
• Document patient care and services accurately in accordance with Medicare/Medicaid and agency policies.
• Identify and report changes in patient conditions, ensuring timely interventions and continuity of care.
• Work collaboratively with other healthcare professionals, including physical therapists, occupational therapists, and social workers.

Qualifications

• Graduate of an accredited school of professional nursing. BSN, PHN preferred.
• Current license as a Registered Nurse in the state(s) of practice.
• Minimum of two (2) years experience in a home health or acute care setting or equivalent experience.
• Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair. Good driving record.
• Current CPR certification.
• Strong communication and interpersonal skills.

Salary : $43 - $47
