Social Worker/Case Manager

(Job Description)

Department: Community Services

Category: Regular Full Time

Reports to: CS Director

Hours of Work: 40 hours

PCN: P404

Level: G-09

FLSA Status: Non-exempt

SUMMARY

This position is responsible for assessing, planning, implementing, monitoring and evaluating actions required meeting the consumer’s needs.

JOB DUTIES

  • Assesses consumer needs and eligibility for services.
  • Coordinates and provides services that are client-centered
  • Develops and maintains client case records, plans and reports.
  • Handles case assignments, drafts service a plan, reviews case progress and determines case closure.
  • Work with consumers and appropriate community agencies to coordinate and facilitate services for consumer.
  • Assists consumer in making informed decisions by acting as their advocate.
  • Attends and testifies at court proceedings, if necessary.
  • Actively participates in team review of records and cases.
  • Provides information to the general public regarding the program
  • Provides case management/file documentation for individual caseload, and submits monthly and quarterly reports in a timely manner.
  • Provides outreach and support services, i.e. home visits.
  • Develops professional relationships with other Federal, State, and Tribal agencies, in order to develop resources for consumers.
  • Performs other duties as assigned.

NECESSARY KNOWLEDGE, SKILLS, AND ABILITY

  • Knowledge of health, social services, legal issues, and other general/local services throughout the Fairbanks area.
  • Ability to learn and apply program/agency operating policies and procedures.
  • Ability to report to work in a timely manner.
  • Skill in operating personal computer utilizing a variety of computer software.
  • Understanding and sensitivity to diverse cultures and lifestyles, and ability to work effectively with Alaska Native populations.
  • Skill in managing multiple priorities and tasks concurrently and meet deadlines.
  • Skill in oral and written communication.
  • Skill in establishing and maintaining cooperative and respectful working relationships with children, parents, supervisors, and other employees.
  • Ability to work independently as well as with teams.
  • Ability to effectively present instruction, and/or information to individuals, staff, administrators, and public.

MINIMUM QUALIFICATIONS (Education & Experience)

  • Bachelor’s Degree in Social Work

PREFERRED QUALIFICATIONS (Education & Experience)

  • Master’s Degree in Social Work
  • Two (2) years’ work experience in the social services field.

WORKING ENVIRONMENT

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

The work is generally sedentary, requiring routine walking, standing, bending, and carrying of items weighing less than 25 lbs. The majority of work is performed in an office setting with a wide variety of people with differing functions, personalities, and abilities. The office is a non-smoking facility with outside smoking areas. The work requires local travel between various program components of FNA which may involve exposure to extreme weather conditions. Works in an environment where protection of consumer confidentiality is essential. The work may require crisis intervention with troubled consumers, which may put the incumbent at risk of physical violence. Personal Protective Equipment will be provided where/when needed.

SPECIFICATIONS

Supervision Received:

Position receives direct supervision from immediate supervisor; however, incumbent is expected to effectively perform day-to-day activity as denoted within job description. Work will be monitored through weekly meetings, monthly reports, and regular communication with your supervisor. General guidelines for operations are based on program requirements and the general policies and procedures of FNA.

Position Impact:

Impact at the program level is high. Program’s level of compliance with program standards is directly affected by the effective performance of duties, especially as they relate to providing program services.

Per FNA’s automotive insurance policy, in order to drive an FNA-owned vehicle, an individual must be at least 21 years of age. In order to drive an FNA-owned vehicle with consumers in the vehicle, an individual must be at least 25 years of age. No employee may transport an FNA consumer in a personal vehicle.

Continued employment status will be contingent upon satisfactory completion of a criminal background investigation and the pre-employment drug testing process. Where required, continued employment status (post job offer) will be contingent upon employee providing certification of annual tuberculosis (TB) screening and medical certification that employee does not, because of infectious or communicable disease, pose a direct threat or significant risk to the health and safety of program participants or others in the workplace.
