What are the responsibilities and job description for the CERTAIN Case Manager position at Family Resources Inc.?
This position provides case management services to youth in the CERTAIN program which focuses on youth between the ages of 5-17 who are at-risk of or currently dealing with truancy issues at school. Case Management responsibilities will include linking the youth and their family to appropriate agency and/or community services; assist in assessing/service planning/service coordination/referral/aftercare services; assist the youth and their families through the recommended services identified.
Level II background and Drug Screen required.
