Serves as a member of the professional team, responsible for the provision of quality programs and services, to assist the individual consumer towards realization of their highest level of independence.
Job Description:
Plans, evaluates, develops and implements case management services towards obtaining the vocational success of assigned referrals.
In a coordinated effort with the consumer and other members of the team, develops individual program plans, assigning each consumer to the program section which best meets the needs of the program goals.
RESPONSIBILITIES:
Maintains work area in a safe manner that assures the safety of the occupants, notifying immediate supervisor of unsafe conditions observed in the facility.
Interprets diagnostic reports of consumer academic, social and vocational abilities, and medical and psychological reports. *
Monitors consumer progress, productivity and attendance; and in conjunction with the consumer, making changes in his/her program plan as necessary. *
Provides individual and group counseling on a planned and as needed basis for agency consumers.
Develops resources for services, materials and/or equipment for individuals with limited vocational skills referred to the facility, or determined ineligible for services.
Performs evaluation and testing procedures for consumer during the 90-day assessment periods. *
Assists with intakes and makes recommendations in program options. *
Performs staffing, referral and follow-up services for consumers in assigned caseload and assisting other staff when needed during months of heavy caseload activity. *
Maintains confidential case records relevant to the consumer, documenting program results, interdisciplinary notes, unusual incidents, etc. *
Compiles necessary information from consumer staffing and related documents for program evaluation. Helps gather data for statistical reports and demographics. *
Attends professional seminars, conferences and training sessions. Attends required staff meetings and associated functions.
Transports consumers to hospital or doctor in emergency.
Maintains telephone and written contact with referring counselor concerning program results and any problems, which may occur. *
Reports immediately to immediate supervisor and the Executive Director if any consumer abuse is indicated or suspected, inside or outside the agency.
Hours: Monday-Friday 7:00am-3:30pm
