Summary:
Responsible for assessing, planning and implementing initial and ongoing training and educational needs. Responsible for clinical orientation and annual competency development and assessment for all nursing areas. He/she will be a clinical resource person for nursing and other related ancillary staff. Facilitates and provides education for clinical and non-clinical courses using external and internal sources including, but not limited to: ACLS/PALS/BLS/Clinical Application training/CPOE/HED/Care Organizer/Portal) in coordination with Human Resources, Information Services, Department Managers and Administration. Assist with needs assessment and training of staff in relation to policy/procedure/protocols/Quality and Performance Improvement initiatives. Availability to work occasionally as a staff nurse to maintain competencies and assess staff needs. Some rotational call, evening and week-end work is required. Some committee involvement will be necessary. This person must be self-motivated and able to work independently. Additional responsibility will include assisting the Clinical Leader with all aspects of the department operating functions with rotational call every other weekend.
The Social Worker is responsible and accountable for assessing the psychological, social, emotional, cultural, and financial needs that impact high risk patient’s health and recovery. The Social Worker demonstrates the ability to utilize social services resources, supportive counseling, and clinical knowledge in the assessment of these needs. Collaborates with the case manager in the development of a discharge plan and addresses assessed needs in a timely manner. The Social Worker will demonstrate a working knowledge of payor source implications on discharge plan, resources, and placement options to maximize a patient’s successful disposition. The social worker provides updates on the psychological, social, emotional, cultural and financial needs to the interdisciplinary team to assist in the delivery of care. Identifies quality and risk issues and reports to the appropriate individual or department. The social worker is also responsible for assisting in the management of metrics including Length of Stay and Avoidable Days. Perform delegated duties consistent with the scope of practice and functions within the philosophy and overall plan of this facility.
Requirements:
Bachelor's degree in Social Work, Sociology, or Psychology from an accredited university. Two years experience in case work or counseling in a health care or social services setting. Experience in an inpatient care setting preferred.

Valid and active social work license preferred
