Duties
- Assist clients with daily living activities, including meal preparation, personal hygiene, and grooming.
- Implement and follow individualized care plans tailored to each client's needs.
- Provide support for individuals with diverse needs, including autism, dementia, and other disabilities.
- Administer medications as prescribed and monitor clients for any changes in health or behavior.
- Facilitate social interactions and community involvement to promote social skills and inclusion.
- Maintain accurate documentation of services provided and progress towards goals.
- Collaborate with healthcare professionals, social workers, and family members to ensure comprehensive care.
Requirements
- Experience in long-term care or home community care is preferred.
- Knowledge of working with individuals with disabilities, including autism and dementia.
- Familiarity with care plans and medication administration protocols.
- Strong interpersonal skills to effectively communicate with clients, families, and team members.
- Ability to work independently as well as part of a team.
