Community Health Worker

The Fund for Public Health in New York City (FPHNYC) is a 501(c)3 non-profit organization that is dedicated to the advancement of the health and well-being of all New Yorkers. To this end, in partnership with the New York City Department of Health and Mental Hygiene (DOHMH), FPHNYC incubates innovative public health initiatives implemented by DOHMH to advance community health throughout the city. It facilitates partnerships, often new and unconventional, between the government and the private sector to develop, test, and launch new initiatives. These collaborations speed the execution of demonstration projects, effect expansion of successful pilot programs, and support rapid implementation to meet the public health needs of individuals, families, and communities across New York City.

Program Overview

Consistent with the Health Department’s decades long commitment to place-based investments, new work is focused on neighborhoods that have been hardest hit by the COVID-19 pandemic, have low vaccination rates, and have been harmed by racial and ethnic health inequities. DOHMH seeks to continue place-based anti-racism interventions toward COVID-19 recovery and chronic disease prevention over the coming two years through continued investment in the Public Health Corps.

Healthy Start Brooklyn (HSB), a federally-funded program based in the DOHMH Bureau of Brooklyn Neighborhood Health, seeks to reduce disparities in maternal, infant, and family health. Serving Central and Eastern Brooklyn , HSB is a five-year grant that supports services, education , and training to enhance the lives of families in these communities harmed by health inequities The grant is awarded by the federal Health Resources and Services Administration (HRSA).

Position Overview

DOHMH is seeking a Community Health Worker/Program Assistant to support Healthy Start Brooklyn. Our work is aimed at reducing health disparities to improve maternal and infant health outcomes, reduce maternal mortality and severe maternal morbidity, and close the gap in race-related infant mortality risk. We work to achieve this aim by offering services to pregnant and postpartum women, their infants, and families, including doula support, health education and training, fatherhood services, and care coordination. Participants are prenatal and postpartum residents of the HSB project area (zip codes 11203, 11206, 11207, 11208, 11212, 11213, 11221, 11233) who are at higher risk for poor perinatal outcomes, including those associated with chronic disease.

The Community Health Worker will participate in home visiting and care coordination, outreach and partner coordination, screening and community assessment, resource navigation, and administrative efforts such as form submission, expense tracking, and inventory management.

Responsibilities

• Conduct outreach, develop relationships, and strengthen collaboration with community residents, organizations, and other stakeholders to improve health equity in Brooklyn
• Coordinate the “ What Now? Postpartum Support from Birth to 18 Months” program by providing home visits, completing ­­­­­­­­­screenings, and offering resource navigation
• Accurately collect and enter data to track progress
• Co-facilitate postpartum support group, and help facilitate other community group efforts
• Support health education class coordination
• Assist with administration of By My Side Birth Support Program (completing client exit interviews, maintaining client files and contact lists, creating client folders for doulas to use, etc.)
• Gather and submit invoices for payment by the Fund for Public Health in New York
• Monitor budget and spending throughout the fiscal year using an expense tracker
• Support purchasing requests by creating purchase orders and coordinating with vendors
• Assist with managing, monitoring, and tracking all programmatic resources distributed to the public
• Participate in quarterly meetings with subrecipients to ensure they are meeting all requirements; complete and submit monitoring documents for federal funder
• Schedule and assist with logistics of meetings hosted by Healthy Start Brooklyn (HSB) and HSB leadership
• Provide support in emergency response efforts, including disseminating COVID-19 and Brooklyn Bureau programmatic information and resources (e.g., PPE) to increase access to and awareness of services
• Support other programmatic needs within the Bureau
• Other duties as assigned by supervisor
  
  

Qualifications

• High school graduation or equivalent and at least three years of experience in community work or community-centered activities in an area related to duties described above
• Quick learner and self-starter
• Able to manage projects and work independently, and within a team
• Understanding of federal grants management and DOHMH policies and procedures a plus
• Some community engagement experience a plus
• Familiarity with data entry and tracking
• Excellent verbal and written skills; presentation skills a plus
• Experience working and/or engaging in priority neighborhoods
• Strong interpersonal skills and the ability to build trust and maintain relationships with clients and community partners
• Strong organizational skills with good follow-through to complete assignments within specified timeframes
  
  

Preferred Qualifications:

• Demonstrated understanding of social determinants of health, institutional & structural racism, health inequities, and their impact on health issues in urban, underserved neighborhoods
• Administrative experience in a health or social service organization
• Outstanding written and oral communication skills
• Bilingual in Spanish or Haitian Creole
• Positive attitude & strong work ethic

35 hours per week

Monday - Friday, 9:30am - 5:30pm, with the possibility of outreach events some evenings and weekends

Hybrid

This position will be funded through June 30, 2025, with the possibility of extension.
