Job Overview

Join Horizon as a Home Health Aide in our esteemed Hospice program, where you will play a vital role in delivering compassionate care to patients in their homes. Your primary responsibility will include assisting patients with personal care activities, supporting their health maintenance, and ensuring a nurturing environment under the guidance of a Registered Nurse or Therapist.

Key Responsibilities

• Provide direct assistance with personal care tasks, such as bathing, dressing, and mobility transfers.
• Review patient care plans prior to delivering services, ensuring alignment with physician’s orders.
• Communicate effectively with patients about the care being provided, fostering their involvement in the process.
• Complete mobile care tasks in accordance with established care plans, ensuring timely and accurate documentation.
• Collaborate with patients, families, and agency staff to address concerns and enhance the care experience.
• Maintain a clean and supportive home environment by executing home maintenance tasks as specified in care plans.

Qualifications

• High School Diploma or equivalent required.
• Valid Wisconsin CNA certification allowing work in Federally Certified Home Health Agencies and Hospices.
• Previous experience in a public-facing role is essential.
• Current and valid Wisconsin Driver’s License, along with reliable and insured vehicle.
