MISSION: Glenkirk will provide quality, lifetime support, and services which empower individuals with developmental disabilities to participate fully in all areas of community life. Glenkirk will advocate for and provide these supports and services based on the principle of informed choice, personal dignity, and individualization thereby enhancing the quality of life for all Glenkirk serves.
BASIC FUNCTION: To provide support to the functions of the Community and Family Supports department, including Intake, Respite, outreach & capacity building, marketing, and special projects.
SCOPE: Provides support to the Community and Family Supports Coordinator to assist with intake functions, including project management of special intake initiatives, participation on the Intake Committee, coordination of candidate visits, and compilation of funding application packets. Assists with the Respite program's administrative functions under the coordinator's supervision. Participates in marketing and outreach for the growth of Glenkirk programs.
WORK PERFORMED:
Intake Functions:
• Assist with intake process functions, including screenings, tours, trials, packet compilation, applications, and transition planning
• Intake Committee Member- screen packets with the team for potential candidates; assist with coordination between divisions
• Participate in Family Intake Meetings prior to admission
• Compile and submit funding packets for intake
Respite Functions:
• Annual Family Enrollment
• Monthly Utilization tracking
• Ensure compliance with DHS/DCFS requirements
RESPONSIBILITY AND AUTHORITY
You will be responsible for assisting with intake and respite functions under the direction of theCoordinator of Community and Family Supports. As well, you will ensure you have all required materials with you on-site to facilitate daily operations. You will be expected to use your personal car in order to conduct home tours, screenings and attend resource fairs. You will be reimbursed for personal vehicle use in accordance with the Glenkirk Travel Expense reimbursement policy.
REQUIREMENTS
Education: BA in human services or a related field required.
Experience: At least one year of experience in the field of intellectual disabilities. Ability to complete accurate screening assessments for individuals regarding their support needs. Proven ability to organize and maintain documentation. Proven ability to write clearly. Proven ability to maintain positive relationships with coordinating agencies. Proven ability to develop positive relationships with families.
Knowledge: Knowledge of DHS-DD services, funding requirements, and criteria for placement. Knowledge of clinical assessment skills, computer word processing skills, project management skills, and time management skills.
Physical Requirements: Ability to communicate verbally and in written form. Ability to commute to various locations within the Chicago Metropolitan area. Ability to read written materials. Ability to communicate verbally and non-verbally with individuals.
