CARE COACH JOB DESCRIPTION

As a Care Coach, your responsibilities will include developing personalized care plans, coordinating with medical professionals and other service providers, and offering compassionate support to our clients. Requirements:
· Capable of making informed decisions based on research and analysis
· Proficient in interpersonal relations and flexible
· Excellent management, organizational, and strategic thinking abilities
· Proficient in computer and technology applications
· Effective communication skills
· Bi-lingual in Spanish and English
· Valid driver's license for in-state travel
· Collaborative team player with a positive attitude, receptive to feedback, and adaptable to changes
· Successful completion of all necessary training
· Dedicated to leading a Christ-centered life
Responsibilities:
· Oversee the admission, referral, and intake processes for new survivors
· Build relationships and establish trust with clients, both in the office and in the field
· Collaborate with clients, their support networks, and healthcare providers to create individualized care plans that address their physical, mental, and spiritual well-being, as well as their preferences and abilities.
· Research and cultivate partnerships with local organizations while keeping an updated resource list
· Attend to clients’ concerns and provide suitable interventions and resources
· Document clients’ progress, including referrals, visits, and notable interactions
· Regularly evaluate clients’ progress and make necessary adjustments to improve outcomes
· Coordinate weekly meetings for internal communication
· Arrange services, and transportation, and maintain records of survivors' progress
· Compile reports by gathering, analyzing, and summarizing treatment data along with relevant statistics and metrics
· Assist with outreach initiatives, speaking engagements, and training sessions
· Enhance job knowledge through educational opportunities; reading professional literature; and maintaining personal networks.
Education:
Master's in social work or related field (preferred) and two years of related job experience. At Glory House we encourage continued education we encourage joining a professional organization that aligns with your responsibility and our mission. Glory House would cover membership. Upon CEO review and approval.
Salary : $58,000 - $61,000
