DEFINITION OF POSITION:The Community Health Worker will work primarily within the community at client homes as well as Emergency Department, healthcare provider offices, hospitalized patients (and with case managers), and with other agencies/organizations to coordinate care and client needs. QUALIFICATIONS:
A. EDUCATION: Required: High School Diploma or equivalent. Valid Oregon driver’s license.Desired: Certification as a Community Health Worker (CHW), Certified Nursing Assistant (C.N.A.) or Certified Medical Assistant (C.M.A.) (strongly preferred). Training in health education or other healthcare field.
B. EXPERIENCE:Required: Prior community outreach or medical/office/healthcare experience. Desired: Prior experience with teaching or one-on-one counseling for health concerns. Previous supervisory experience desirable. In-home visitation experience and phone follow-up experience recommended.Capable of functioning in a fast-paced environment with strong interpersonal skills. Ability to communicate effectively in oral and written form. Ability to assist and support creative, independent actions for clients. A well-organized self-starter who works well independently, as well as a team member. Previous group or one-on-one teaching experience.
C. EQUIPMENT/TOOLS /WORK AIDS:General knowledge of computer skills (Word, EXCEL, Outlook, Publisher), telephone systems, and copy/fax machines. Knowledge of community health information resources and how to access. Knowledge of required software programs helpful or able and willing to learn software. Trained on the use of tablets/Skype for patient documentation or willing to learn.
D. SPECIALIZED KNOWLEDGE AND SKILLS:Required: Proficiency in English (speaking, written and understanding). Proficiency in Spanish bilingual/bicultural in all areas of communication: speaking, written (strongly preferred). First Aid/CPR certification or obtained prior to seeing clients.Communication, Cultural Competence, Ability to work collaboratively, Basic understanding of anatomy/physiology and clinical procedures. Willing to become certified as a community health worker and maintain knowledge in the following areas: Medicare, Oregon’s Health Insurance Marketplace (“Marketplace”), Medicaid and other State Assistance programs, community resources and services, duties/roles of community health worker/and or non-traditional home workers.
E. Working Conditions Primarily inside environment, protected from weather, but not necessarily temperature changes. Occasionally subject to hostile or angry clients. Subject to exposure to infectious disease, hazardous materials and equipment. Variable noise levels. Also subject to rapid pace, multiple stimuli, unpredictable environment and critical situations. Must be able to drive distances in all weather to patient homes, provider offices, area meetings and to interagency collaborative partner facilities.
F Physical Demands/Traits Ability to sit at a desk, or to stand and work continuously for extended period of time. Able to lift up to 50 lbs. Ability to travel to various sites throughout the surrounding communities. Flexible hours. Able to access and work in external and internal settings.
G OTHER: Proficiency speaking, writing and understanding English required and proficiency in Spanish (written, speaking, understanding) strongly desired.
