Job Summary
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will be responsible for providing comprehensive case management services to clients enrolled in the mental health and substance use disorder (MH and SUD) program. This includes assessing client needs, coordinating care, linking clients to resources, and advocating for their well-being. The Case Manager plays a critical role in supporting client retention and engagement throughout their treatment journey.
Key Responsibilities
- Client Assessment and Planning:
- Conduct initial and ongoing assessments of client needs, strengths, and challenges.
- Develop individualized case management plans in collaboration with the clinical team and clients.
- Monitor client progress and update plans as needed.
- Care Coordination:
- Serve as the primary point of contact for clients, ensuring they have access to necessary resources and support.
- Coordinate referrals to community services, housing, employment support, medical care, and other social services.
- Work closely with therapists, medical providers, and peer support specialists to ensure seamless service delivery.
- Advocacy and Support:
- Advocate for clients' needs with service providers and other organizations.
- Educate clients about available resources and how to navigate systems of care.
- Provide crisis intervention and support as needed.
- Documentation and Compliance:
- Maintain accurate and timely case notes, client records, and documentation in accordance with state and program requirements.
- Ensure all services provided are documented and tracked for compliance and reporting purposes.
- Participate in audits and quality assurance processes to ensure program standards are met.
- Group Facilitation:
- Plan and lead structured group sessions focusing on life skills, recovery strategies, and wellness planning.
- Foster an environment of mutual respect and peer support within group settings.
- Collaborate with the clinical team to ensure groups align with treatment goals and program objectives.
- Client Retention and Engagement:
- Develop strategies to enhance client retention and engagement in the program.
- Conduct regular check-ins and follow-ups to ensure clients remain connected to care.
- Identify barriers to participation and work with clients to address them.
- Collaboration and Teamwork:
- Participate in interdisciplinary team meetings to discuss client progress and care plans.
- Provide input on program improvements and client service delivery.
- Support the clinical team in achieving program goals and objectives.
Qualifications
- Bachelor's degree in Social Work, Psychology, Counseling, or a related field.
- Case Management certification or other relevant credentials.
- 1-2 years of experience in case management or a related role (experience in MH and SUD outpatient programs preferred).
- Knowledge of local community resources, social service systems, and best practices in case management.
- Familiarity with ASAM criteria and electronic health record (EHR) systems preferred.
Job Type: Full-time
Pay: $20.00 - $25.00 per hour
