Social Worker
The Grand Rehabilitation and Nursing in Utica is seeking a full time Social Worker for our skilled nursing facility.

Duties Include:
- Responsible for assessments, care planning & helping residents adjust to their new surroundings.
- Discharge planning experience preferred
- Familiar with a variety of the field's concepts, practices & procedures.
- Relies on extensive experience & judgment to plan and accomplish goals.
- A wide degree of creativity and latitude is expected
- Occasional Community Outreach
Requirements Include: Candidates must have:
- A current/valid Bachelors Degree.
- Long-Term Care exp. preferred
- Exceptional interpersonal & leadership skills
- Knowledge of applicable state and federal guidelines.
