Responsibilities

The Nurse Care Manager serves in an expanded nursing role to collaborate with patients and their primary care providers to offer a model of care that ensures the delivery of quality, efficient, and cost-effective healthcare services.

The Essential Functions of the Nurse Care Manager :

  • Assesses, plans, implements, coordinates, monitors and evaluates all options and services with the goal of optimizing the patient's health status.
  • Integrates evidence-based clinical guidelines, preventative guidelines, protocols, and other metrics in the development of treatment plans that are patient-centric, promoting quality and efficiency in the delivery of healthcare for the populations.
  • Develops systems of care that monitor patient progress and promote early intervention in acute care situations.
  • Works effectively with other members of the health care team to optimize interventions.
  • Manages utilization and practice metrics to further refine the delivery of care model to maximize clinical, quality, and fiscal outcomes for identified populations.

The Nurse Care Manager, RN, Family Medicine is located at the Grand View Health Family Practice in Harleysville.

Qualifications

Education and Experience :

Vocational Nursing Diploma, and over1 to 3 years of office RN experience required

Associate's or BSN preferred

Licenses and Certifications :

Registered Nurse in PA – Licensed, upon hire, required

Basic Life Support (BLS), upon hire, preferred
