DUTIES AND RESPONSIBILITIES :
The Primary Care Provider (PCP) delivers comprehensive primary care by actively collaborating with other healthcare team members, like behavioral health specialists, to address a patient's physical and mental health needs holistically, focusing on coordinated care and addressing social determinants of health within a single practice setting. PCPs provide the full scope of primary care services, including diagnosis, treatment, coordination of care, preventative care, and health maintenance to patients. They serve as leaders and the point of contact for a patient's overall health while facilitating communication and collaboration across the care team to ensure that each patient gets the best possible care.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Participates as an active member of the medical staff and participates in daily care team huddles to ensure prioritization of patient needs.
- Attends to patients in a timely and orderly manner to keep waiting time to a minimum and maximize productivity.
- Conduct initial evaluations, diagnose medical conditions, and develop treatment plans for patients.
- Obtains appropriate patient history, performs physical evaluation, orders appropriate diagnostic tests and follows-up with interpretation, formulates assessment and initiates treatment plan with appropriate follow up appointments.
- Reviews incoming reports (such as lab, x-ray, EKG) signs, dates and follows up with patients in a timely manner.
- Screen patients for mental health concerns, provide brief counseling sessions, and refer to specialized behavioral health providers when necessary.
- Collaborate with other healthcare team members, including nurses, social workers, and specialists, to ensure coordinated care and seamless transitions between services.
- Provide health information and education to patients regarding their conditions, treatment options, and preventative care strategies.
- Ensures documentation is complete in a timely and orderly manner, utilizing the electronic medical record.
- Actively participate in team meetings to discuss patient cases, share information, and coordinate care plans.
- Adheres to GCLC’s productivity and quality standards and meets minimum requirements.

LEVEL AND DEGREE OF SUPERVISION
Supervised by the Medical Director and direction from the consulting pharmacist and Nurse Practitioner. Supervision to include one (1) hour per week and participation in case staffing, general, and team meetings.
