The Guidance Center is Hiring a Care Manager Near Flagstaff, AZ

POSITION SUMMARY:
The primary function of this position is to provide services for seriously mentally ill (SMI) clients through coordination efforts with other clinical team members, supervised/independent living, and others within the community. Home and community visits are an expectation of the position. This position will be expected to maintain an acceptable level of productivity (60%) as measured by billable hours/service value. The focus of this position is to provide frequent intervention and support to stabilize this population.
ESSENTIAL FUNCTIONS
1. Coordinates and provides services as indicated on the Service Plan. Document all services in the client record.
2. Coordinates and facilitates the access to and the delivery of services to the individual. These services are provided on a walk-in basis, at scheduled appointments, by telephone, and in person in the client's home or other community setting.
3. Assists with coverage of a peer's case load as needed.
4. Meets with each client as appropriate per service planning guidelines and individual client severity.
5. Provides immediate intervention and initial screening for individuals needing stabilization and crisis counseling (i.e. utilization of CSU, PAC Unit, clinic staff and psychiatrist).
6. Provides consultation and education to families concerning the treatment of the client.
7. Works with community agencies in implementing work adjustment training, social skills development, and leisure time through vocational rehabilitation, etc.
8. Adheres to case management standards per HCIC, State and JCAHO by maintaining documentation of clients' charts in a timely manner.
9. Participates in service activities as required and makes self-available for community activities.
10. The employee must work well under pressure meeting multiple and sometimes conflicting deadlines.
11. Collaborate cooperatively with colleagues and supervisors.
12. Performs other duties as assigned
KNOWLEDGE/SKILLS/ABILITIES

Required:

  • Master's degree or bachelor's degree in a field related to behavioral health or one of the following: Bachelor's Degree and at least one year of full-time behavioral health experience; Associate's Degree and at least two years of behavioral health experience; High School Diploma and four years' experience in the behavioral health field; High School Diploma and two years' experience in the behavioral health field and 18 credit hours in Sociology, Social Work, Psychology or a closely related field completed in the previous four years.
  • Must possess or be able to obtain a State of Arizona Level One Fingerprint Clearance Card
  • Current Driver License with no driving restrictions
  • Minimum age 21 years old
  • Ability to work flexible hours
  • Able to travel to other clinic sites if needed
  • Ability to work as part of a team to coordinate client care
  • Strong interpersonal skills
  • Complex problem- solving skills
  • Technical skills with computer and able to demonstrate competency
  • Resource management skills
  • Adult teaching skills

Preferred:

  • One year of case management experience
  • Prior experience working with the SMI population
  • Experience providing crisis services
  • Knowledge of community resources available, including benefits and entitlement process
  • Understanding of managed care
  • Trained in crisis intervention
  • Familiar with SMI diagnoses, treatment and special needs
  • Familiarity with commonly used psychotropic drugs and their side effects
