Role Overview
Join our team as a Case Manager for the Youth Kauhale Program, where you'll play a key role in supporting young adults experiencing homelessness on their journey to independence. You'll manage cases, coordinate with a diverse team, and ensure participants receive the services they need, including housing, medical care, and employment resources. Your work will involve conducting assessments, developing service plans, advocating for clients in meetings, and connecting them to vital community resources. This is a dynamic role where you'll have the opportunity to make a meaningful impact on the lives of young people while working in a collaborative and supportive environment. Qualifications :
- Bachelor's Degree in Social Work, Psychology, or related Human Services field
- At least one (1) year of experience with youth and youth services is required
