Summary of Job Duties:
- Serves as a Family Support and Care Coordinator, guiding individuals and families through systems of care by providing prevention resources, triage, referrals, and follow-up support.
- Responsible for developing and delivering training programs for internal staff and community partners.
- Establishes and maintains partnerships with first responder agencies, community providers, and stakeholders.
- Oversee certification processes for training programs, such as OPATA certification for QRT participants.
- Maintain accurate participant records in the Cordata system and track progress against key performance indicators.
- Bachelor’s degree in public administration, business administration or a human services field such as Social Work, Psychology, Special Education, Therapeutic Recreation, Sociology, Criminal Justice, Human Services (including a clinical practicum of at least one year), or a related field from an accredited college or university; AND
- One (1) year of human services-related experience
