Position: Community Care Field Supervisor
Location: Harlingen, Texas
Job Summary:
We are seeking a highly motivated and dedicated individual to join our team as a Community Care Field Supervisor. The Community Care Field Supervisor will be responsible for overseeing and coordinating the delivery of community care services to individuals in need. This role involves providing leadership and supervision to a team of community care workers, ensuring that all services are delivered in accordance with company policies and guidelines.
Responsibilities:
- Supervise a team of community care workers, providing guidance and support as needed
- Conduct regular evaluations and assessments of client needs, ensuring that services are tailored to meet individual requirements
- Develop and implement care plans in collaboration with clients and their families
- Monitor and track client progress, making adjustments to care plans as necessary
- Ensure that all services are delivered in a timely and efficient manner
- Coordinate and communicate with other healthcare professionals, social workers, and agencies to provide comprehensive care to clients
- Maintain accurate and up-to-date records of client interactions and care plans
- Provide training and support to team members to ensure high quality care delivery
- Participate in staff meetings, training sessions, and other professional development activities as required
Qualifications:
- High School degree or general equivalency diploma in social work, nursing, or related field
- 2 years of experience in a healthcare or social services setting
- Strong leadership and organizational skills
- Excellent communication (English & Spanish) and interpersonal abilities
- Knowledge of community care services and resources
- Ability to work independently and as part of a team
- Valid driver's license with current vehicle liability insurance and access to a vehicle for travel to client locations
