Role Description
This is a full-time on-site role for a Hospice Nurse at Heart of Georgia Hospice. The Hospice Nurse will be responsible for providing nursing services in a home care setting. The role involves ensuring comfort, dignity, and support for hospice patients and their families.
Key Tasks/Responsibilities
- Managing their schedules and managing patient care
- Educating the patient and/or the patients’ caregiver(s) about hospice and hospice care.
- Coordination with IDT (Social Workers, Chaplains, Supervisors, and the Director of Clinical Services).
- Completing all necessary nursing paperwork/EMR documentation in a timely manner.
- Required 2nd call shift rotation typically once every 6 weeks, includes weekends and holidays, maybe subject to change to meet patient needs.
- Must be willing to assist during an emergency.
- Performs other tasks as assigned by the Clinical Manager.
Qualifications
- Strong communication and interpersonal skills
- Compassionate and patient-centered care approach
- Active RN license in the state of Georgia
- A minimum of one year of RN experience
- Current RN licensure for the state of Georgia
- Valid driver’s license.
