Job purpose

The Psychiatric Registered Nurse (RN) position is key to ensuring follow up with patients of Heartland’s psychiatric department.

Duties and responsibilities

• The Psychiatric RN is the first point of contact for patients referred from the LMH Health Emergency Department/Integrated Crisis Team, or from other community agencies. The Psychiatric RN is responsible for scheduling patients, informing patients of Heartland policies and fees, and providing reminder calls
• The Psychiatric RN is responsible to lead the daily huddle: bringing information about patients and their last contacts with the practice, possessing a depth of knowledge of patients to understand patient’s current status and challenges, review any recent contacts the team has had with patients
• Coordinate monthly treatment team meetings for patients with other members of patient’s care team to help enhance patient outcomes
• Provide health-related education and resources to patients on issues such as symptoms of mental illness, med compliance, and basic management of other health conditions that might impact mental and overall health, both in office and through between-visit phone follow up calls
• Maintain contact with patients who are hospitalized, participate in discharge planning, and communicate with providers involved regarding after-care plans
• Make targeted phone calls to follow up with patients who are engaged in psychiatry services, behavioral health consultant services, and those patients participating in Medication Assisted Treatment (MAT)
• Follow up on medical information (DM mgmt., for example), as well as information re: to psychiatric symptoms and collaborate with patient’s primary care provider and care team as needed
• Use a system which is decided on at the end of the visit re: to placement on an acuity team to indicate level of acuity and frequency of follow up patients require
• Use tracking protocol (excel sheet, care management software in eCW, as determined by program staff) to track follow up and progress
• Complete documentation in eCW consistent with Heartland’s policies and procedures manual,
• Refer to BHCs assertively
• Follow up on BHC visits as well as psychiatry visits
• Help patients get connected to resources in the community for SUD, intensive mental health services, and other resources as appropriate
• Work with the MA to complete medication reconciliation with pharmacies when a patient is not doing well, or in a targeted way to confirm what medication a patient is taking and if medications have been picked up from the pharmacy
• Coordinate with other specialists' offices as needed
• Other special projects and duties as assigned

Qualifications

• The Psychiatric RN is required to have the RN license, BSN is preferred
• At least one year experience in an outpatient or inpatient mental health setting
• An understanding of psychiatric disorders and their presentations in a diverse outpatient setting
• Knowledge of psychiatric medications and side effects
• Ability to learn technology systems, generally technology-savvy
• Strong verbal and written communications skills and ability to communicate ideas successfully
• Ability to remain calm when under pressure
• Good computer skills and knowledge of Microsoft programs

• Regular and predictable attendance as scheduled is an essential function of the job

Working conditions

• The Psychiatric RN is located in a busy medical clinic
• The incumbent will be faced with constant interruptions and must meet with others on a regular basis

• Job tasks are performed in close physical proximity to other people
• Requires working indoors in environmentally controlled conditions
• Includes exposure to disease or infections

Physical requirements

• Requires sufficient visual acuity to be able to read and write and operate equipment common to this position
• Required to read, write and speak English
• Requires sufficient hearing level to be able to effectively communicate with people directly or by telephone
• Occasionally lift items weighing up to 25 pounds

• Hours M – F from 8:30 am – 7 pm as needed. (approx. 40 hours per week)
