What are the responsibilities and job description for the Outreach Case Manager, Homeless Services position at HELP of Ojai?
HELP of Ojai’s Community Assistance Program provides immediate and long-term help to low-income families, individuals and older adults that reside in the Ojai Valley. The Homeless Services Outreach Case Manager will work directly with clients living in the City of Ojai’s homeless encampment and provide supportive services during the transition to permanent housing and beyond.
Responsibilities:
· Conduct client intake and personalized needs assessment, collaborating to create a service plan that focuses on housing goals, income maintenance/increase, and other client-identified personal goals such as medical, mental health, substance use, financial, vocational, and social support needs.
· Provide referrals, connections, information, and support to resources that assist clients in achieving their service plan goals.
· Complete specific program assessments (e.g., VI-SPDAT) through the Homeless Management Information System.
· Assess strengths and obstacles related to short-term and long-term goals.
· Adhere to documentation standards as required by the Coordinated Entry System and HELP of Ojai, including timely completion of case file notes and maintaining a complete, accurate, and current client file with all necessary documents.
· Ensure client file confidentiality in accordance with HIPAA and other applicable guidelines.
· Collaborate effectively with various stakeholders, including clients, community partners, service providers, and internal team members, to support client success and program goals.
· Possess a deep understanding of the challenges faced by homeless individuals and other marginalized populations in crisis.
· Actively listens to participants and engage in effective problem-solving strategies using trauma-informed communication techniques.
· Provide referrals for emergency services, including crisis housing, drop-in centers, domestic violence support, human trafficking assistance, sexual assault services, mental health care, and extreme weather relief.
· Attend and participate in staff meetings, case conferences, training workshops, and community meetings as required.
· Assist in audit preparations by retrieving relevant data.
· Work independently, organize and prioritize tasks, and meet critical deadlines.
· Handle multiple tasks with competing priorities effectively.
· Be adaptable and open to new information, changing conditions, and unexpected obstacles.
· Exercise sound judgment when answering questions, releasing information, and analyzing and projecting the consequences of decisions or recommendations.
· Maintain a high level of ethical conduct, particularly regarding confidentiality, dual relationships, and professional integrity.
· Assist other HELP of Ojai staff when needed.

Qualifications:
· Requires a minimum of a bachelor’s degree in Psychology, Social Sciences, or related field.
· Two (2) years of experience in healthcare, case management, homeless services or working with vulnerable populations or a related field.
-Experience entering data and tracking outcomes within HMIS preferred.
· Computer literate with Microsoft Office, internet, and Google Drive.
· Bilingual English/Spanish preferred and ability to perform all duties in both languages as needed.
Working Conditions:
· Case Manager will spend at least 50% of their time in the field
· Other locations include office site, West Campus, and other sites as needed.

Additional Information:
Work hours are Monday - Friday, 8:30-5:00 although there will be some nighttime and weekend hours on occasion to attend community events, conduct early morning or late-night collaborative outreach efforts, or attend to emergent client and administrative needs.
Full Time, Hourly Non-Exempt, 40 hours per week. Starting salary is $25 - $30 per hour with medical and dental benefits provided, and generous paid time off. Position begins immediately. This is not a remote position.
