What are the responsibilities and job description for the Case Manager position at Hiawatha Behavioral Health?
The ideal candidate will have the following responsibilities:
Promote recovery and community integration for adults with mental illness, respecting their right to person centered planning and least intrusive alternatives, while also providing support services. Responsible for assessment, treatment planning, monitoring, linking and coordination for person served. Performs necessary liaison, advocacy and consultation work with ancillary agencies such as: Department of Health and Human Services, Courts, Foster Care Providers, Guardians, and Families. Provide periodic reviews to ensure appropriate services are being provided, and treatment plans are reflective of services. Responsible for continuous development of treatment plans, providing guidance, and advocating for person served.
To be successful, this candidate must have:
1. Bachelors degree required in Social Work, Psychology, or other related Human Service field.
2. State of Michigan Registration as Social Service Technician, or the ability to obtain within one year of employment.
3. Knowledge and/or experience assisting people diagnosed with substance use disorders.
4.Proficient computer skills necessary, with experience utilizing Excel & Microsoft Word programs.
5.Appointment is subject to driver’s license and criminal background check. Must be at least 18 years of age, possess a valid driver's license, and provide own transportation.
