Primarily responsible to provide services that meet the social and emotional needs of residents, primarily in the nursing home unit. Participates in the development of each resident’s comprehensive care plan under the direction of an Licensed Social Worker (LSW).
Provides psychosocial assessments and grief therapy for Hill View residents and their families.
Is directly involved in completing each resident’s Plan of Care under the direction of an LSW.
Provides technical assistance to Hill View’s Admissions staff on social work issues as identified across the Continuum of Care.
Makes referrals to and provides liaison support to the Behavioral Medicine Medical Director. Assists in the transfer of residents to In-Patient psychiatric treatment centers when needed under the direction of an LSW.
Provides education to Hill View staff in appropriate behavioral interventions. Develops in-service training of staff related to psychiatric assessment/behavioral/dementia appropriate interventions with the Health Care Director of Nursing and other team members.
Assists in the development of policies pertaining to psychiatric and behavioral concerns.
Provides information/educations and guidance to families seeking guardianship of a resident who is incompetent under the direction of an LSW.
Provides communication/education regarding Power of Attorney responsibilities/resident rights.
Completes assessments of residents, writes reports, and completes Geriatric Depression Scales, Mini Mental Evaluations or other appropriate assessment tools as per doctor’s orders and signs off on progress notes under the direction of an LSW.
With clerical support makes sure appropriate documentation is in place per regulatory requirements and meetings are scheduled per regulatory requirements.
Provides any related administrative record keeping and management services as directed in an organized, accurate and timely manner in accordance with regulatory requirements, including maintaining grievance log.
Follows care principals of social work, following professional, ethical and regulatory guidelines and processes as defined by the profession under the direction of an LSW.
Make sure MDS interviews for Hill View health care residents are completed in a timely and appropriate manner under the direction of an LSW.
Completes discharge planning according to regulatory requirements under the direction of an LSW.
