Home Health Physical Therapist

We are currently looking for a licensed physical therapist for part time full time or PRN to visit patients in their home and provide home exercise program, safety evaluation, and promote and prevent falls and improve strengthening, ROM and muscle education.

We receive referrals in the following areas: Streamwood, Elgin, West Dundee, South Elgin, Pingree Grove, Surrounding suburbs.

If you enjoy working with one patient at a time and improving their overall health, safety and mobility this role is for you. We are looking for a dynamic and motivational therapist who sets goals for the patient/family and works with our interdisciplinary team to achieve clinical outcomes.
