Job Overview: The Targeted Case Manager Supervisor is responsible for overseeing and guiding a team of case managers who assist individuals in achieving their personal goals and improving their overall quality of life. This role requires a strong understanding of case management, leadership, and the ability to work with diverse populations, ensuring that clients receive targeted and individualized care.
Responsibilities:
- Supervise and manage a team of targeted case managers, providing guidance, support, and regular feedback.
- Conduct performance evaluations and assist in the development of staff training and professional development.
- Ensure the implementation of case management protocols, adhering to relevant laws, policies, and ethical guidelines.
- Monitor and review case plans, client progress, and services to ensure effective and appropriate interventions.
- Provide regular coaching and support to case managers to improve performance, handle challenging cases, and resolve client concerns.
- Collaborate with community agencies, healthcare providers, and other service organizations to ensure comprehensive care for clients.
- Assess client needs and assist in the creation of individualized care plans.
- Assist with recruitment, onboarding, and retention of case management staff.
- Ensure accurate and timely documentation of client interactions, assessments, and progress notes.
- Maintain knowledge of current trends and developments in case management practices and services.
- Handle escalated cases or concerns and work directly with clients to resolve any issues.
- Participate in meetings and report to senior management regarding team performance, case load, and outcomes.
Requirements:
- Bachelor's degree in Social Work, Psychology, Human Services, or a related field (Master's preferred).
- Minimum of 3-5 years of experience in case management, with at least 1 year in a supervisory role.
- In-depth knowledge of case management practices, especially in targeted case management for vulnerable populations.
- Strong interpersonal and communication skills, with the ability to work effectively with clients, families, and community partners.
- Proven leadership and team management abilities.
- Ability to work independently and manage multiple priorities in a fast-paced environment.
- Proficiency in client management software and Microsoft Office Suite.
- Familiarity with state and federal regulations governing case management services.
- Bilingual in English (and additional languages are a plus).
- Certification or licensure in case management or related fields may be required or preferred.
- A compassionate, patient, and solutions-focused approach to client care.
- Must be a Certified Behavioral Health Case Manager Supervisor (CBHCMS).
