What are the responsibilities and job description for the Hospice Social Worker - Full Time position at Hospice of Southwest Ohio?
Hospice of Southwest Ohio is looking for a full time licensed Social Worker to join our team. The social worker provides psychosocial and emotional support to the terminally ill patient and their family/caregiver and friends, promoting comfort and optimizing quality of life. The social worker assists the hospice patient and their family/caregiver in difficult decision making. Flexible schedule with some on call duties required.
Knowledge of Hospice Care and the psychosocial issues of terminally ill patients and their families. Excellent communication skills and the ability to coordinate care well with an interdisciplinary team.
Knowledge of terminal illnesses, medical terminology and pain assessment. Knowledge of Medicare, Medicaid and Commercial Insurance reimbursement.
Knowledge of community resources to address patient/family needs.
MSW from accredited school by the Council on Social Work Education required.
