Job Summary:
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Hospice Inpatient Facility team. In this role, you will provide high-quality, patient-centered nursing care to patients with life-limiting illnesses and their families. Our ideal candidate will have a strong passion for hospice care, excellent clinical skills, and the ability to work collaboratively within an interdisciplinary team to ensure comfort and dignity during the end-of-life journey.
Key Responsibilities:
- Deliver skilled nursing care to patients in accordance with the established care plan, hospice guidelines, and regulatory requirements.
- Conduct thorough assessments of patients' physical, emotional, spiritual, and psychosocial needs to develop and adjust individualized care plans.
- Administer medications and treatments as prescribed, monitor patient responses, and educate patients and families about medication management and symptom control.
- Collaborate with physicians, social workers, chaplains, and other team members to ensure comprehensive care and support for patients and their families.
- Provide emotional support and counseling to patients and families, assisting them in coping with end-of-life issues and decision-making.
- Document all patient interactions, assessments, and care provided in a timely and accurate manner in the electronic health record.
- Participate in family meetings to discuss care plans, goals of care, and end-of-life options, ensuring effective communication and family involvement.
- Assist in the orientation and training of new nursing staff, providing mentorship and guidance as needed.
- Adhere to infection control and safety protocols to maintain a safe and healthy environment for patients, families, and staff.
- Participate in ongoing staff education and quality improvement initiatives to enhance the hospice care experience.
Qualifications:
- Current and valid Registered Nurse (RN) license in Georgia.
- Associate Degree in Nursing (ADN) with relevant experience.
- Strong clinical assessment skills and the ability to manage complex patient care scenarios.
- Excellent communication and interpersonal skills, with a compassionate approach to patient care.
- Ability to work collaboratively within a multidisciplinary team environment.
- Strong organizational and time management skills, with the ability to prioritize tasks effectively.
- Basic Life Support (BLS) certification required; Advanced Cardiac Life Support (ACLS) certification preferred.
- Knowledge of hospice philosophy, policies, and regulations is a plus.
