Housing Works established New York State’s first harm reduction-based, OASAS-licensed outpatient drug treatment program. Other services include Health Home care management, behavioral health, and syringe exchange programs. Housing Works fights for funding and legislation to ensure that all people living with HIV/AIDS have access to quality housing, healthcare, HIV prevention, and treatment, among other lifesaving services.

Benefits:

We have three comprehensive healthcare plans to choose from based on your priorities and budget. Housing Works covers most of the plan; you pay a portion, based on your salary. Staff begins accruing PTO immediately for a total of up to 30 days earned in the first year. We offer employees an educational benefit. This money is available for tuition loan reimbursement, tuition costs, and text books.

Compensation Range: $20-$22 an hour with commensurate experience

Summary Description:

The Care Manager works with clients and the client’s network of providers to help minimize barriers to care and help clients achieve improved health outcomes. Using interventions such as care coordination, motivational interviewing and health promotion, care managers help clients, over the course of time, develop the skills needed to manage their health and psychosocial needs with greater independence. Care Managers will have the opportunity to work as part of outcome driven integrated teams and participate in agency wide quality improvement activities aimed at improving the delivery of care to individuals living with chronic illness, behavioral health issues, and homelessness.

Responsibilities:

  • Provides comprehensive care management, care coordination and health promotion that is safe, timely, effective, and efficient.
  • Completes comprehensive health assessment/reassessment.
  • Develops, reviews, and revises individual person-centered care plans alongside client.
  • Links/refers to social, health, community, and governmental benefits, services, and agencies as needed.
  • Conducts client outreach and engagement activities to assess on-going and emerging needs.
  • Promote continuity of care and improved health outcomes
  • Documentation in EMR System
Minimum Requirements

Qualifications/Requirements:

  • Bachelor’s Degree plus 2 years related experience preferred. Masters Degree in related field may be substituted for 1 year experience.
  • Computer literate and working knowledge of Microsoft Office (Word, Excel)
  • Good interpersonal skills and ability/interest in working as part of a team.
  • Knowledge of HIV, substance use, homelessness, mental illness, and other chronic medical conditions
  • Knowledge of City, State, and Federal entitlement systems is helpful.
