Summary
Under the direction of the Case Management Supervisor, the Case Manager is responsible for providing crisis intervention, advocacy, accessing the needs of residential clients, and implementing case plans for individuals and/or families whose lives have been impacted by relationship violence. The Case Manager is responsible for providing supportive services to clients that focus on securing permanent housing, employment, and childcare while utilizing best practices of trauma informed care. The Case Manager will work closely with community partners to ensure clients have access to resources and services that will further support self-sufficiency, safety, and housing security.
Duties and Responsibilities

Assess the financial, social, and emotional needs of the client.

Work with clients to create a case plan that focuses on short-term and long-term goals; meet with clients to review, assess, and support progress.

Advocate for the client to community partners, programs, and within the agency.

Collaborate and develop community partners, social workers, and other service providers to secure community resources for the client.

Serve as a support to families in crisis and complete reassessment of needs.

Conduct screenings for prospective clients and arrange emergency hotel stays, as appropriate.

Arrange emergency transportation for clients, as appropriate.

Complete intake paperwork with new residential clients.
Qualifications

Bachelor's degree in human services, related field, or commensurate work experience required.

English and Spanish fluency preferred.

Two (2) years' of experience in case management with families is preferred.

One (1) year of experience working with at risk children and families is preferred.

Valid CA Class C license and auto insurance, and/or ability to travel to meetings, trainings, and home visits.

Complete post-hiring Domestic Violence 40-hour training.
