Reports To: Vice President of Quality and Risk
Status: 80 hours per two week pay period
Shift: 8:00 am – 4:30 pm
Job Schedule: Full Time
Weekly Hours: 40 hours
Remote Position: No

Job Summary
HRMC is seeking a self-motivated, resourceful and forward-thinking individual to lead our Social Services team. Works to provide exceptional social services consulting for HRMC.

Qualifications: (Minimum Education and Experience)
- MSW required.
- Licensed by the South Dakota Board of Social Work Examiners.
- Visual, auditory and speech acuity requisite to verbal and written communication with staff, patients, families, physicians and other persons with whom contact is part of the work activities.
- At least two years' experience in mental health or dialysis.
Physical Aspects of the Job/Essential Physical Demands:
- Work is indoors in clean, well-lighted, climate controlled environment with minimal hazards under normal conditions. Works in patients' homes, hazards subject to condition of patients' homes.
- Involves travel to patients homes.
